Role-Specific Documentation

Each provider type has slightly different documentation expectations. Use the cards below as a quick checklist before starting your application or onboarding.
Travel Nurses
  • Updated resume and complete employment history.
  • Active state nursing license and background check documentation.
  • Government-issued ID (such as driver license) and Social Security card.
  • Immunization records, including required vaccines and annual tests.
  • A recent physical exam and any other facility-specific documents requested.
Travel nurses may also be asked to provide skills checklists, specialty certifications, and proof of specialty experience.
Licensed Nursing Personnel
  • Roles include NPs, APRNs, CRNAs, RNs, LPNs, CNAs, MAs, and ancillary staff.
  • A reliable email address you monitor consistently.
  • Current resume, skills documentation, and prior employment details.
  • 1–2 hours allotted to complete the online application fully — pause and resume as needed.
  • Application must be completed within 3 weeks. Incomplete applications are removed after two months.
Physician Assistants
  • Education and training documentation such as program completion or residency records.
  • Professional credentials including diploma, board certification, BLS/ACLS.
  • DEA and state licenses; name-change documentation if applicable.
  • Three professional references familiar with your clinical work.
  • Documents can be provided electronically via secure upload.
Compliance

Health, Immunization & Safety Compliance

To protect patients and staff, all clinicians must meet health screening, vaccination, and safety training requirements in line with facility and regulatory standards.
Core Immunizations & Medical Tests
  • Hepatitis B vaccination or documented immunity.
  • Measles, mumps, and rubella (MMR) immunization.
  • Varicella vaccination or titer.
  • Tetanus, diphtheria, and pertussis (Tdap) vaccination.
  • Current tuberculosis screening with negative results.
  • Additional immunizations as required by facilities (e.g., COVID-19).

Begin the immunization and testing process early — some results may take several days to finalize.

Safety, Background & Professional Fitness
  • Verification against exclusion, sanction, and restricted-party lists.
  • Criminal background history check, where required.
  • Drug screening in line with facility and assignment policy.
  • Fitness-for-duty verification based on physical exam and job demands.
  • Ongoing OSHA and safety-related education as required.

Credentialing confirms your identity, qualifications, and professional standing through verification of training, licensure, experience, and any applicable sanctions or privileges.
Steps to Apply
  • Complete the online application using a reliable email address that you check regularly.
  • Upload or submit your resume, licenses, certifications, and required health documentation.
  • Provide accurate employment history and professional references so your experience can be verified.
  • Respond quickly to follow-up questions or additional document requests from the credentialing team.
Setting aside one to two focused hours helps you complete most of the process in a single sitting and prevents delays.
Behind-the-Scenes Verification
  • Credentialing reps review your application and compare it with your CV for consistency.
  • Prior employers or facilities where you have worked may be contacted to verify standing.
  • Any licensure sanctions, malpractice claims, or disciplinary actions are reviewed as required.
  • Foreign-trained providers may be asked for ECFMG certification and immigration documentation.

Credentialing for Physician Assistants

Documentation to Prepare
  • Complete the online application using a reliable email address that you check regularly.
  • Upload or submit your resume, licenses, certifications, and required health documentation.
  • Provide accurate employment history and professional references so your experience can be verified.
  • Respond quickly to follow-up questions or additional document requests from the credentialing team.
Setting aside one to two focused hours helps you complete most of the process in a single sitting and prevents delays.
Professional References
  • Provide at least three professional references.
  • At least two should be clinicians within your specialty.
  • Include peers who have worked with you within the last 24 months.
  • Ensure all contact information is current and accurate.
Employers want to hear about your overall competency from your peers or supervisors.
Medical Tests Required
  • Hepatitis B.
  • Measles, mumps, and rubella (MMR).
  • Varicella.
  • Tetanus, diphtheria, and pertussis (Tdap).
  • Meningococcal disease.
  • Negative tuberculosis test result within the past year.
Full Disclosure
  • Report all licensure sanctions or malpractice claims in detail.
